Job summary

We are looking to recruit a trainee Advanced Care Practitioner to join the In-patient Rehabilitation Teams as part of Hertfordshire Community NHS Trust. You will work as part of the multidisciplinary teams across our 3 bed bases contributing specialist skills and expertise in the delivery of the patient's medical care.

The service consists of 3 units across Hertfordshire:

Danesbury Neurological Centre, specialising in the rehabilitation of patients who have an acute neurological diagnosis including stroke. The Queen Victoria Memorial (QVM) Hospital, specialising in the rehabilitation of patients who have a variety of health conditions including falls, fractures, amputees, frailty and bariatric care. Herts & Essex Hospital, specialising in a similar cohort to QVM.

The successful candidate would be expected to complete a Masters accredited university course to become a fully qualified ACP. We would also welcome applications from practitioners who have completed a non-accredited pathway/course but who are willing to complete the e-portfolio route to become a fully accredited ACP. Individuals who have partially completed some relevant Masters levels modules will also be considered for a condensed/personalised pathway.

Main duties of the job

During the training period of this post, the post holder will increasingly learn to apply advanced clinical expertise, levels of judgement, discretion and decision making within the in-patient units; this will involve placements across all 3 Hertfordshire Community Trust in-patient units, as well as other relevant healthcare environments. The post-holder will work under decreasing supervision & with increasing independence to act as an autonomous practitioner, exercising expert clinical practice in assessment, diagnosis, prescribing and development of medical treatment plans for patients with complex presentations and multi pathologies.

The post holder will increasingly learn, with decreasing supervision required, to be an expert resource, provide a visible clinical profile, have both advisory and clinical input into patient care within the in-patient units; this includes provision of expert professional advice to patients, carers and colleagues, and ensuring the maintenance of clinical excellence.

The post-holder will work collaboratively with the multi-disciplinary team including medical and allied health staff to (with supervision) manage medical conditions and to plan and implement treatment for illnesses.

As a Trainee Advanced Clinical Practitioner, this post-holder will provide learning to other staff and students within the clinical area.

Job responsibilities

Work under supervision to learn to provide autonomous expert clinical practice in clinical clerking of the patient on arrival to the unit including assessment, diagnosis and development of medical treatment plans, for a range of patients with acute, non-acute, and chronic medical conditions. This will include analysis of symptoms, initiation of interventions IV cannulation and ordering of diagnostic tests and x-rays as required, then interpret and adjust treatment regimens as appropriate. Work under supervision to learn to recognise the rapidly deteriorating patient and provide clinical interventions and prescribe the medical management plan according to clinical needs. Makes clinical decisions on transfer of acutely unwell patients, and liaises with receiving medical team. Collaborate with Doctors/Consultant and work under supervision to learn to plan and implement treatment for complex and multi-pathology illnesses, including managing a community inpatient caseload on a daily basis, undertaking independent ward rounds adjusting care as required.

Qualifications and Training

Essential

Currently Registered with the Nursing & Midwifery Council First Degree or equivalent experience Teaching and Assessing Qualification GCSE Level 4 or equivalent in Maths & English

Desirable

Post-Graduate - Extended Independent and Supplementary Appropriate PostRegistration specialist education / qualifications or equivalent, examples could include (not exclusive) in elderly care, long term conditions, neurological care, rehabilitation, medical care etc Advanced Life Support

Experience and Knowledge

Essential

Needs significant post registration experience commensurate with being an Advanced Clinical Practitioner and demonstrate competence and experience in clinical management. Minimum of 5 years post registration experience Minimum of 2 years at band 6 in a relevant specialty Evidence of leading organisational and management change

Desirable

Experience of Advanced Life Support skills
