Job summary

The cardiac rehab service is looking for a Senior physiotherapist with an interest and skills in Cardiac Rehabilitation.

The cardiac rehab service is part of the Integrated Community cardiology service, which provides diagnosis and management of heart failure, cardiac rehab phase 2 and 3, and GP with specialist interest cardiology clinics.

You will work with the cardiac rehab nurses, associate practitioners, assistant practitioner/exercise instructors and volunteer, to provide cardiac rehabilitation to patient either in group settings, via remote home-based programmes and will provide home visits where required.

The physiotherapist's focus will be on the assessment monitoring of functional outcomes, supporting quality improvement where needed, and individual tailoring of programmes where other conditions require adjustments

This role is very much working with our highly experienced nurses who will be able to support with nursing and heart failure management.

This is a new role in the service which we are excited to add to the team, to improve our MDT offer for patients. The service has recently started to participate in National Audit Cardiac rehab, and we are aiming for accreditation in 12-18 months.

Once in post, you would be expected to undertake student practice educator training and supervise students.

Due to the community nature of the role, a car driver with access to a car for business use is essential.

Main duties of the job

To work as part of the integrated community cardiology team to deliver nursing/therapeutic input to patients with the aim of returning them to maximum independent living, within a stimulating environment actively promoting self-management on all activities of daily living. Working as an autonomous practitioner, you will provide specialist input to patients in the cardiac rehabilitation caseload. Work with colleagues across health, social care, mental health and voluntary sector to ensure holistic and joined up delivery of care. Empower patients to make decisions about their own care and encourage them to exercise their rights and choices, signposting them to relevant community and voluntary services and supporting patients carers and assistants with management of risk.

Person Specification

Experience

Essential

Specialist physical and manual skills and techniques relevant to care group Supervise the work of students, assistants and volunteers Proven experience working in the relevant care group.

Desirable

Recognised registered qualification and further courses and experience relevant to role Competencies relevant to specialism Experience of managing the work of others

Qualifications

Essential

Registered physiotherapist with HCPC Relevant post qualification training through short courses and/or further study and experience for example splinting and injections, working towards post graduate level of study. Evidence of on-going CPD

Desirable

Registered member of Chartered Society of physiotherapy Member of BACPR

Knowledge

Essential

Professional knowledge and Skills acquired through a combination of formal training, short courses and experience to degree level or equivalent. Good theoretical and practical knowledge of a broad range of assessments including specialist relevant to the specialist care group and the relative benefits of each. Good knowledge of a range of specialist therapeutic interventions relevant to the care group including the relative benefits of each and the comparative outcomes. Awareness of national guidance relevant to own specialities, including, consent issues, and other legal frameworks relating to care group Understanding of the roles of other professionals. Knowledge of standards of record keeping. Knowledge of the current evidence based practice
