Job summary

An exciting opportunity has arisen for an enthusiastic, proactive registered nurse with experience in Multiple Sclerosis or Neurology to join the MS Senior Nurse Neurology at GRH. This is a fixed term 5 month ( Band 6 post to cover maternity.

The neurology team based at Gloucestershire Royal Hospital provides a patient-centred service to those with acute and chronic neurological illness, in both inpatient and outpatient settings.

You will be expected to exercise a high degree of professional autonomy and to make critical judgments using specialist knowledge. You will have responsibility for assessing patients and subsequently devising, implementing and assessing an individual's comprehensive MS care plan.

The post holder will need to possess excellent communication and interpersonal skills and is required to work closely with the MS nursing team, consultant neurologists and health colleagues within primary and secondary care, social care and voluntary agencies, contributing to and implementing care for people PwMS.

A background in neurology and/or neurosurgery is essential. Whilst MS experience is desirable, training, support and development will be provided to the appropriate candidate who can demonstrate the motivation, interest and drive to deliver a high level of specialist intervention and care to patients with MS and those that support them.

Job responsibilities

- To provide specialist nursing advice and support to patients, their families and other healthcare professionals following diagnosis and through treatment through a range of nurse-led outpatient clinics including annual reviews, relapse and relapse follow up and telephone clinics.

- To provide specialist nursing advice and support to patients, their families and other healthcare professionals following unplanned hospital admissions to hospital, to co-ordinate care, liaising with the full range of multi-disciplinary team members to ensure implementation of appropriate care plans, treatment delivery, and safe and effective discharge

- To follow-up patients as appropriate and ensure all relevant information is available to enable patients to make an informed choice about their treatment.

-To manage nurse led clinic and independently conduct neurological examinations and objective measures of function such as assessments of cognition, fatigue, upper limb function or walking.

- Assess patients with acutely deteriorating symptoms and diagnoses relapse in routine and non-routine circumstances using high level assessment and consultation skills and ensure timely and appropriate treatment is offered.

- Competently reviews and interprets data generated through these assessments and revises the care plan in conjunction with the person with MS accordingly.

- To develop, deliver and evaluate a plan of care relating to symptom management which will include assessment, planning, implementing, reviewing and documenting care plans in agreement with the person with MS. This will include but not exhaustive of ordering diagnostic tests and making and receiving referrals.

- To support PwMS with decisions regarding disease modifying therapies (DMTs).

- To ensure all patients are fully informed of the risks and benefits of proposed treatments, contributing to the process of securing informed consent.

- To produce and provide all patients with relevant and accessible written information regarding their treatment in accordance with Trust standards.

- Provide a telephone helpline and maintain clinical databases.

- Deliver audit and the coordination of research studies.

- Ensure that the service successfully meets national, local and departmental quality and clinical standards.

You may be asked to support other specialty service such as PD, MND, MS, and Epilepsy team when required.

Experience of developing advanced practice guidelines, Governance and audit.

Qualifications

Essential

Registered Health Care professional NMC/HCPC. Independent Prescribing Non-Medical Prescribing Qualification. Experience of research-based practice, and quality improvement through audit.

Desirable

Recognised post graduate management/ leadership qualification, working towards one or willingness to undertake. Enhanced IT skills - working knowledge of software packages such as word, excel and outlook alongside EPR, PACS, ICE etc Ability to utilise information from databases and computer-based systems in order to evaluate and develop the quality and efficacy of service delivery.
