Job summary

Care Plus Group Primary Care practicesprovides the first point of contact for registered patients accessing thehealth care service. The service will work collaboratively with patients toassist them in managing and preventing illnesses and coordinating access tospecialised care.

The Care Plus Group Primary Care practices,Open Door and Quayside, provide services for a client group with a range ofdiverse and sometimes complex needs including substance misuse, homelessness,mental health problems and self-neglect.

Main duties of the job

Job Title: Outreach Healthcare Assistant

Department: CPG Primary Care

Hours: hours Monday Friday

£22,370 Per Annum

Fixed term for 12 months

This 12-month position will be based within Open Door and Quayside Medical Centre however will be an outreach position. The post holder will undertake NHS health checks in a variety of different places such as community settings, factories, supermarkets and patients own home to work on the completing of NHS health checks for the whole of North East Lincolnshire. The post holder must be able to undertake general healthcare assistant tasks such as bloods and blood pressures.

The HCA will undertake a range of delegated clinical roles as agreed by the registered nurses and General Manager including regular computer skills along with the ability to organise and prioritise a varied workload are necessary. The post holder will also be required to act in a professional manner at all times and adhere to practice policies, protocols and guidelines.

The post holder must have undertaken a healthcare assistant role previously.

Essential

Ability to work with minimum supervision Effective and proven computer skills Ability to work as part of a wider team An awareness of own competencies and willingness to seek guidance where needed Ability to maintain clear and accurate records both electronically and in writing, to deadlines Ability to communicate clearly and sensitively with people who have a wide range of healthcare needs. Ability to maintain confidentiality Ability to demonstrate empathy and sensitivity Ability to deal effectively with confrontational situations To have a range of clinical skills, such as, wound care, eye care, pressure area care, venepuncture, blood pressure monitoring and blood sugar monitoring Ability to demonstrate commitment to Care Plus Group core values

Qualifications

Essential

QCF level 3 in Health and Social Care Level 2 qualification in Numeracy and Literacy or equivalent level of competence.

Flexible approach to working arrangements. Ability to transport self and equipment around the North East Lincolnshire area, within prescribed timescales
