Job summary

The Ethnicity, Mental Health Improvement Programme (EMHIP) is a three-year innovative locality-based service improvement programme consisting of five key interventions to reduce ethnic inequalities in access, experience and outcomes of mental health care in South West London.

It is a collaborative partnership, South West London ICS, South West London and St Georges Mental Health NHS Trust and Wandsworth Community Empowerment Network (WCEN) and local BME communities.

The postholder will be responsible for managing the Crisis Family Placement Scheme of EMHIP and delivering the various aspects of this intervention.

Main duties of the job

The post holder will be required to undertake additional duties appropriate to their role and in the ongoing development of the CFP scheme. Managing core elements of the project, the key responsibilities include:

Selection and placement of guests in ensuring and managing the placement plans, in partnership with SWLSTG clinical teams and CEN EMHIP teams.

Liaison with Host Families to support their role, support the placements, and working with the Wandsworth Home Treatment Team, and associated IRH's to ensure a safe and effective care plan.

Supporting, monitoring, facilitating, and developing the scheme's progression, and promoting the scheme within mental health services, and allied agencies to propose changes which would impact across the acute care pathway.

Initiating, developing, and Implementing policies and processes to manage the service and leveraging project management tools and methodologies.

Providing expert support in identifying and implementing improvement schemes, utilising risk logs, action plans, and highlight reports.

Be responsible for collecting, maintaining all relevant data, placement information and ensuring these are consistently collected and monitored.

Ensuring all improvement schemes adhere to quality, safety, and benefits realisation metrics, with thorough risk management practices in place.

Ensure project plans are robust and updated on progress identifying key risks and issues.

Job description

Job responsibilities

Recruit host families and engage with potential candidates from the public and community organisations. Collaborate with SWLSTG and CEN EMHIP teams to manage guest placements. Lead the guest selection process and ensure safe, effective placements with host families. Support and supervise host families, working with the Wandsworth Home Treatment Team and CMHTs. Maintain regular communication and coordinate training for host families. Monitor, facilitate, and promote the scheme within mental health services and allied agencies. Develop and implement policies, processes, and improvement schemes using project management tools. Collect, maintain, and monitor relevant data and placement information. Ensure adherence to quality, safety, and benefits realisation metrics. Develop robust project plans, identify key risks and issues, and provide regular updates. Lead change management efforts and support project/programme teams in achieving milestones. Liaise with guests to involve them in presentations and conferences. Attend management and clinical meetings and facilitate discussions for host family referrals. Be responsible for maintaining confidentiality pertaining to sensitive information of Host Family and guest records. Person Specification

Post Graduate Diploma /Project management qualification or equivalent. Project management qualification or equivalent

Related qualification in nursing/ or mental health and social care
