Job summary

RedhillPhoenix Primary Care Network is a busy and vibrant organisation, consisting ofthree practices in Redhill, Surrey. Between the three practices they providehigh quality care to a population ofaround 28,000 patients.

This roleis based at Holmhurst Medical Centre, where a clinical pharmacist is sought tolead on repeat prescribing at the practice, as well as a large variety of othertasks, with excellent supervision and opportunities to participate in projectwork from time to time.

The role is embedded in the practice but thesuccessful candidate would also benefit from being part of a much wider team ofclinical pharmacists across the four Primary Care Networks who sit underDorking Healthcare.

Main duties of the job

The ClinicalPharmacist will have a key role in supporting delivery of the Network ContractDES Service specifications. For the new Structured Medications Review andOptimisation requirements this will include tackling over-medication ofpatients, including inappropriate use of antibiotics, withdrawing medicines nolonger needed through NHS-led programmes such as low priority prescribing andmedicines optimisation more widely. For Enhanced Health in Care Homes residentswill benefit from regular clinical pharmacy-led medicines reviews.

The PCNsare critical to the changing NHS and are the focal point of all locallydeveloped planning and service delivery.

Qualifications

Essential

Mandatory registration with the General Pharmaceutical Council Independent prescriber

Skills and Aptitudes

Essential

Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions Is able to obtain and analyse complex technical information Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to senior managers or GPs when appropriate Excellent interpersonal, influencing and negotiation skills Excellent verbal and written communication skills Organised, good time management skills and can meet deadlines with minimum supervision Positive approach, calm under pressure Good IT skills, sufficient to use Word, databases and spreadsheets Can produce timely and informative reports Demonstrates accountability for delivering professional expertise and direct service provision to patients Has a can do attitude to daily tasks, adversity and new ways of working Undertakes healthy working with regards to achieve the most efficient way to achieve objectives and tasks

Desirable

Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences ( patients) Persuasive, can gain acceptance of recommendations and is able to influence/motivate/persuade others to comply with recommendations/agreed course of action where there may be significant barriers

Other Requirements

Essential

Willing and able to travel to different DHC/GP sites and to attend meetings, training events etc. Able to work in the evenings and at weekends.

Car driver with current UK driving licence

Essential

Minimum of 2 years post-qualification experience Experience of working in a similar role before

Knowledge

Essential

Specialist knowledge acquired through post-graduate diploma level of equivalent training/expertise An appreciation of the nature of GP practices An understanding of the nature of primary care prescribing and strategies for improving prescribing

Desirable

In-depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare
