Bridge CottageSurgery are looking for a full time or part time PracticeNurse or Nurse Practitioner to join our supportiveand friendly team of nurses with opportunities for self development.

Bridge Cottage Surgery isa semi-rural practice of approximately 16,000 patients based in Welwyn,Hertfordshire. The surgery has 7 GPPartners, 3 Associate Clinicians, Pharmacist and a nursing team of 4 nurses andan HCA. Bridge Cottage Surgery iseasily accessible off the A1M and has excellent transport links in and around Hertfordshire and is only 20 minutesfrom London.

To providenursing services to a definedpractice population with the assistance of, and in collaboration with the Lead Nurseand GP colleagues.

Main duties of the job

This role will includeclinical duties: ECGs, dressings, BPs, Ear irrigation, injections, cervicalscreening, restocking and ordering. To contribute in the development ofservices provided by thePractice and to participate in clinicalaudits. To work with the GPs todevelop and implement the Practice DevelopmentPlan, the NationalService Framework targets, Key PerformanceIndicators and to meet andmaintain the quality indicators contained within the Quality and OutcomesFramework.

Bridge CottageSurgery is a semi-rural practice of approximately 16,000 patients based in Welwyn, Hertfordshire. We have 7 GP Partners, 3 AssociateClinicians, Pharmacist and a nursing team of4nurses and an HCA.

We have a staff car park, NHS Pension and organise staff events.

Job description

Job responsibilities

1. To work with the Nursing team to assess, plan and providehealth care and nursing services to the practice population, ensuring they are effective,relevant, evidence-based and of a high standard

2. To workwith the Nursing team toprovide a comprehensive generalpractice nursing service, to includewound care, urine tests, childhoodimmunisations, seasonal influenza,pneumonia, shingles, rotavirus immunisations, NHS assessments, ECGs, blood pressure,travel immunisations and advice, within the surgery and where appropriate in patient homes, reviewing bloodtest results and taking action where required.

3. To provide a womens health service, to include cervical screening and advice and information regard breast screening,Chlamydia testing and family planning

4. To with theNursing Team & GPcolleagues to achieve and maintaincervical screening targets. To record and monitor result,including efficient and accurate call and recall systems.

5. To work with the Nursing team to providean effective health promotion service, to includesmoking cessation and healthy lifestyleclinics.

6. To work with child health, to include the administration of the childrens immunisation programme according to current schedules, the accurate recording of the information in the child health records and to participate in measures(such as education sessions) to improve the uptake of childhoodimmunisations, especially the Measles, Mumps and Rubellavaccine, whooping cough in pregnancy.

7. To work with the practice team in order to organise, implement and administer the annual influenza and pneumococcalvaccination programme, to include the identification, targeting andimmunisation of vulnerable patients within the practice population, to ensurethat local and national targets are met.

8. To work with the Nursing team to identifyand maintain registers of thosepatients with chronicdisease such as asthma, chronicobstructive pulmonary disease (spirometry), diabetes, thyroid disease,epilepsy and coronary heart disease, hypertension.

9. To assistin the development, implementation, delivery and evaluation of programmes of care for those patientswith chronic disease as dictated by the National ServiceFrameworks (NSFs) targets and to meetand maintain the quality indicatorslaid down in the nGMS contractinitiated in April 2004

10. To ensure that professionalresponsibilities are met aspractitioner supplying and administeringmedicines in accordance with a Patient Group Direction.

11. To assess patientunderstanding of treatment proposals,gain valid informed consent and works with the NMC Code ofProfessional Conduct to providecare for patients who lackthe capacity to consent to treatment.

12. To usea range of verbal and non-verbal communication tools, to communicate effectively with patients to deliver effective healthcare. This will include patients whohave special communication needs, , thosepatients who are sensory impaired or who have mentalhealth issues.

13. To work with the practice team to ensure clinical governance standards are met, including managing clinical riskwithin own clinics.

14. To workclosely with the practice team, primary care and community nurses, allied health professionalsand social services colleagues by activeparticipation in clinical meetings, to ensure effective lines of communication in order to achieve highstandard and continuity of care for the practicepopulation.

15. To work withthe Nursing team and GP colleagues to develop local enhanced services, if required.
