Case Manager – Community Services (RNLD)
Location: South West (Somerset, Cheltenham, Bristol, Bath, Gloucestershire, Wiltshire)
Salary: £50,000 per annum

Key Responsibilities:
- To support the MedGen Community team with service delivery of care packages.
- Manage and maintain a caseload of clients within the community.
- To build positive relationships with the clients and their family’s or wider networks.
- To thoroughly understand the clients and family’s needs and ensure the abilities of the staff working within the package match the care plan.
- Support new members of staff to be introduced to the package of care through meet and greet with service user and families.
- To be the lead person responsible for shadow shifts and training shifts.
- To role model, supervise and assess staff competencies and build capability through coaching.
- To identify learning needs within the package and communicate to the appropriate teams.
- To fully support with any investigations following an incident or a complaint.
- To identify and report environmental and personal changes in the home environment which may be seen as a risk to both staff and the client.
- Effectively communicate with the Registered Manager and Clinical Lead on a regular basis to ensure good management, quality and safety within the package.
- Support the wider team to ensure all shifts within the package are appropriately covered and to provide emergency cover when required.
- To complete weekly PPE stock takes to ensure stock levels remain at a satisfactory level.
What We’re Looking For:
- RNLD qualification with current NMC registration
- Minimum 2 years’ experience in community care or a similar setting
- Coaching experience (Train the Trainer qualification preferred)
- Strong understanding of CQC Regulations and Social Care Codes of Practice
- Confident with IT systems and documentation
- A proactive team player with excellent relationship-building skills
- Full UK driving license and willingness to travel within the South West region
