An exciting opportunity has arisen for an experienced andmotivated Practice Nurse to join the friendly and forward-thinking nursingteam at Bunbury Medical Practice.We are looking for a highly skilled, autonomous PracticeNurse who is confident in managing long-term conditions, undertakingcervical cytology and delivering childhood and adult immunisations. You willplay a key role in delivering high-quality, patient-centred care to ourregistered population of approximately 5,500 patients.You will work closely alongside Advanced NursePractitioners, GPs, HCAs and the wider multidisciplinary team, in asupportive environment that values clinical expertise, continuity of care andprofessional development.Main duties of the job The following arethe core responsibilities of the Practice Nurse.Assess the needs of patients ensuring the provision of healthcare isappropriate, incorporating evidence-based practiceImplement and embed health promotion and wellbeing programmesImplement and evaluate individual treatment plans for chronic disease patientsthat promote health and wellbeingIdentify, manage and support patients at risk of developing long-termconditions, preventing adverse effects to patients healthProvide routine nursing care to patients in accordance withclinical based evidence, NICE and the NSFDiagnose and manage acute and chronic conditionsPrescribe medication for patients in accordance with PGDs and own scope ofpracticeReview patient medications to enhance complianceProvide chronic disease clinics, delivering patient care asnecessary, referring patients to secondary/specialist care as requiredMaintain accurate clinical records in conjunction with extant legislationToensure compliance with the practice chaperone policyToassist GPs with minor surgery when requiredTosupport the team in dealing with clinical emergenciesToimplement vaccination programmes for adults and childrenTosupport the clinical team with all safeguarding matters, in accordance withlocal and national policiesTodeliver opportunistic health promotion where appropriateAbout us Bunbury Medical Practice is a welcoming, high-achieving rural practice located in a picturesque village near Tarporley, with the added benefit of an onsite dispensary.We are led by 2 GP Partners and 4 Salaried GPs and are proud of our strong, multidisciplinary clinical team, which includes:Advanced Nurse PractitionersPractice Nurses and HCAsFirst Contact PhysiotherapistDementia NurseWe are also an active member of the Rural Alliance Primary Care Network, working collaboratively with five neighbouring practices to enhance patient care and staff development.Job responsibilities To be responsible for the implementation of processesfor the effective management of patients with long-term conditions usingevidence-based practice including care for elderly and housebound patients.Working as a member of the practice multidisciplinaryteam, the post holder will ensure nursing services are delivered effectively tothe entitled patient population.The Practice Nurse will be responsible for a number ofclinical areas such as infection prevention and control, health promotion, chronic disease management, well womenand well man clinics, as well as actively supporting the practice managementteam in the reviewing and delivery of clinical policy and procedure.Bunbury MedicalPractice must ensure that the post holder has access to appropriate clinicalsupervision and an appropriate named individual in the organisation to providegeneral advice and support on a day-to-day basis.Person Specification Qualifications Registered Nurse (Nursing and Midwifery Council)Nurse PrescriberExperience Experience working within a primary or secondary care environmentExperience of chronic disease managementExperience of working autonomouslyExperience of working as a practice nurse or community nurseExperience of infection prevention and control measuresDisclosure and Barring Service Check This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.#J-18808-Ljbffr
