Southern Health NHS Foundation Trust plays a key role in supporting the discharge and ongoing care of patients from our local acute hospital. In order to improve the patient experience we have been asked to provide support to a programme of working relating to Medically Fit for Discharge patients in the South East area of Hampshire areas.

The multidisciplinary Enhanced Recovery and Support at Home team based at Havant and Gosport are looking to recruit Health Care Support Workers to become integral part of our rehabilitation and re-enablement service covering a wide area.

Working with us you will:

- Be part of a lively team providing varied day to day, patient focused short term packages of care, working eight hour shifts over a seven days between the hours of 8am and 9 pm.
- Support our extensive team of OT’s, physiotherapists and nurses in providing community based support usually, although not exclusively, to the over 65 age group within their own homes.
- Be supported in your Care certificate and to attend training courses as part of your ongoing professional development.

We are a large Trust and we have some fantastic career opportunities.

You could go on to Band 4 Associate Practitioner roles or undertake Nurse training to become qualified.

You will need to have access to a car as essential for this role and mileage will be paid monthly.

**Salary**: £20,270.00-£21,318.00 per year
