**Person Specification**
We are looking for an individual who is committed to the values of care and believes in giving people the best and outstanding care in the comfort of their own home.
We need a motivated, dynamic and organised person who can manage and prioritise their own workload according to demand and somebody with a keen eye for detail.
A calm and confident manner is essential for this role.
Due to the structure of the Company, you will be required to travel between locations in order to meet the needs of its staff, service users and the business.
A full, clean driving licence is essential, and an Enhanced DBS check will be carried out.
The Assessment officer is responsible for the provision of holistic client needs assessments, support planning, and referral and information services to prospective and current clients of the Delta Care Ltd. The position aims to assist clients to maintain an active and independent as possible.
**Key objectives include**:
- To perform duties as an Assessment Officer with a focus on the Wellbeing, independence and Reablement. To undertake home/ hospital assessments that are responsive to client’s goals and care needs. This is achieved by accurately and comprehensively completing assessments using Delta Care Ltd support plans and risk assessments.
- To focus on finding the service solutions to best support everyone’s aspirations to maintain and strengthen their capacity to continue with their activities of daily living, social and community connections.
- To develop and maintain systems and protocols that reflect the Wellness and Reablement philosophy, ensuring that the strengths and capacity of clients are captured.
- To foster a coordinated approach to the provision of services and support to clients by establishing excellent professional relationships with external service providers.
**Key Responsibilities and duties**
- Respond to enquiries and referrals from social workers, local authorities and individuals who wish to use Delta Care care services.
- Arrange, conduct and document Home Support Assessments in a timely manner
- Arrange, conduct and document Home Support reviews, in a timely manner.
- Keep accurate records using the company care system. Write up care plans and ensure they are put in the home as soon as is practicable.
- Respond to telephone calls and requests for visits in a timely manner
**Other duties within the role**:
- The role requires some on call duties at weekends and evenings shared between the office team.
- Spot checks on care staff.
- Supervisions of staff if required
**The following knowledge and skills are required in the post**:
- Demonstrated experience in working with older people, people with a disability and their carers.
- An understanding and sensitivity to the needs of people from a culturally and linguistically diverse background.
- The ability to operate a personal computer and an understanding of managing and maintaining databases.
- Ability to manage personal workload understand Delta Care Ltd policies whilst maintaining a customer service focus.
- An ability to analyse problem/difficult situations and to seek solutions that effectively address the issue and are resource efficient.
- Good communication skills
- Flexibility and reliability
This list is not exhaustive but a guide to what is required. The assessment officer will work with the Registered Manager of each office to determine what and when reviews are required.
You will work closely with and support the Manager with assessing complex care packages and care assessments. You may be required to shadow staff when required in relation to complex care packages.
You will keep accurate records and training plans and provide these to Management when required.
