Senior Practitioner North West Bed Bureau/oaps

This is an exciting Band 7 role, based in patient flow team and will support and promote effective patient flow within the Out of Area placements and North West Bed Bureau, which are beds based in the independent sector. The role focuses on individual service users care pathways whilst ensuring least restrictive practice and optimizing lengths of stay and support the GMMH Trust wide goal to reduce out of area placements.
To liaise closely and effectively with key staff in other agencies RAID, CMHT, HBTT, EIT, Later Life, LD, and Rehabilitation Services etc. to maximize safe and timely discharges
To act as a resource to provide education, advice and support relating to all aspects of admissions and discharge planning and provide supervision and training as required to junior members and students of the GMMH Trust Wide Patient Flow Service.
Participate in relevant GMMH Trust Wide Patient Flow meetings
The post holder will attend the local bed management meetings and IS review, contribute to team meetings and keep accurate records.
The post holder will participate in the local Bronze on call rotas as required

Clinical
- Provide clinical expertise regarding admission and discharge criteria and ensure bed management policies and procedures are adhered to.
- To agree transport arrangements including number of escorts required as necessary for service user transfers to and from the inpatient units.
- Work in conjunction with existing care coordinators to facilitate discharge.
- Complete referrals to CMHT for individuals requiring Care Coordinator and maintain liaison with relevant services in the interim.
- Develop links with housing provider services ensuring that procedures are implemented: for example homelessness.
- Developing links with independent service providers.
- Attend CPA, MDT and Ward Round Meetings as required.
- Ensure inpatient CPA pathway is implemented for all service users, inpatient CPA care levels are reviewed and updated as appropriate and discharge CPA’s including crisis plans are completed at point of discharged and communicated with key individuals in the absence of a care coordinator.
- Maintain an up to date working knowledge of Mental Health Legislation, CPA procedure, crisis care planning, safeguarding and risk management.
- Ensure KPI’s, and relevant CQUIN and or CQC targets are completed and maintained.
- Undertake clinical assessments as necessary, including patients placed in other hospitals (NHS and private providers).
- To support and participate in Care & Treatment Reviews (CTR) following admission of service user with a diagnosis of learning disability.
- Undertake assessments on service users in hospital who may benefit from early supported discharge and to explore this option with the MDT, service user and their carer where appropriate.
- Supporting the inpatient service to make post discharge follow up arrangements and complete 48 hr. and 7 day follow ups as required
- Complete referrals to HBT and CMHT services as required to facilitate early and supported discharge from the inpatient services.
- Undertake risk assessment and risk management to an exemplary standard to promote patient safety in line with Trust Policy and Procedure
- Maintain timely and accurate clinical records including inputting data into clinical information systems and local recording systems.
- Participate in supervision, appraisal and training required to maintain clinical competence and professional registration as required.
- Engage in Team meetings
- Engage in daily huddle meetings ensuring that demand and capacity is communicated effectively between the Hub and Spoke Practitioners as well as the whole Trust
Communication
- Act as a point of contact and co-ordinate updates and reviews of patients in IS beds admission to inpatient services.
