**JOB PURPOSE**:
The Community Social Prescriber will have a dual role, providing personalised support to a caseload of community-based patients whilst conducting community development activities across Coventry and Warwickshire. [SH1]

To support clients across Coventry and Warwickshire within the community to improve their level of wellbeing by engaging in community assets. They will coordinate community activity in line with key priorities for the Coventry and Warwickshire Integrated Care Board. This will involve working with Primary & Secondary care, Allied Healthcare professionals, and community sector organisations.

The role will require managing your own caseload whilst conducting community capacity building work and asset-based community development work. It will require proactive engagement with healthcare professionals to stimulate referrals into the programme.

Engagement at all levels is essential, as is relationship building to embed a successful and productive service. The role involves proactive working with healthcare professionals and partner organisations to stimulate referrals into the programme. This will need to take place on a place-base and systemic level, so the role holder will work with other health providers across Coventry and Warwickshire.

Fundamentally, the role will be a point of contact for social prescribing and community health capacity building across Coventry and Warwickshire. This involves supporting the Head of Social Prescribing and other community social prescribers.

**KEY DUTIES AND RESPONSIBILITIES**:

- Day to day community development activity. Supporting social prescribers to understand community health inequalities on a placed-based level.
- To lead on the administration and coordination of community activity across Coventry and Warwickshire (and surrounding areas) within the ICB’s place remit.
- To Lead on community mapping activities across Coventry and Warwickshire, and where appropriate support with how gaps in community provision are addressed.
- Communicate and liaise with community resources and external activity partners to ensure they are aware and fully engaged with the service.
- To lead on and shape specific community health projects.
- Undertake initial assessments; identify goals and develop personalised plans with clients.
- Ensure all work complies with contractual monitoring and evaluation requirements.
- Work with other health professionals, community organisations, voluntary sector, public health, and local charities to build relationships and increase awareness and access to social prescribing.
- Develop comprehensive knowledge of wider support services for people with high support needs including social isolation, wellbeing, housing, unemployment, welfare, and benefits.
- Extend and develop the menu of services prescribed in the service directory
- Maintain effective relationships with Health and Social Care professionals, Information, Advice and Guidance Services, Local Charities, Voluntary Sector, support services, stakeholders, and partners
- Following a needs assessment recognise people’s communication needs and adapt accordingly
- Adhere to Health Exchange’s policies and procedures at all times including Equality and Diversity, Health and Safety, Confidentiality, Lone Working and Safeguarding
- Ensure the Social Prescribing service reflects the vision, mission, and values of Health Exchange
- Undertake any other additional tasks as reasonably deemed appropriate by the Head of Social Prescribing
- To attend relevant meetings to discuss the progress of the project.
