An opportunity has arisen for an Admissions and Discharge Facilitator to become part of the admission and discharge team based at Lings Bar Hospital. The role will support ward/unit based care teams to safely and timely admit and discharge patients into and out of our care. The role will cover working at Lings Bar hospital across all 3 wards and our other Community bed based services with Nottinghamshire locality. We would consider full / part time hours to meet the needs of the post. You will be an active partner in a system wide Integrated discharge process, with care home staff, local GP's and members of the wider multidisciplinary team, social services, the CCG, patients, and their families; therefore, excellent communication skills are essential. A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010. You will have daily responsibility for the patient flow of admissions and discharges of patients within our bed based services. This forms part of the D2A agenda and primarily provides for the Pathway 2 and pathway 3 patients. Working as part of an on site team with the full MDT, you will be expected to optimise and facilitate admissions and discharge planning. The main aim is to ensure patient flow through our services is effective. Part of the role will be to work with the Transfer of Care Facilitator and in reach to NUH to assess the referred patients and ensure they are streamed to the right bed based service, supporting the discharge into community services. In line with the integrated hub approach; There will be a requirement to provide an "in-reach" service as part of this role. This will entail the ADF visiting and supporting the continued assessment of patients within an acute bed based facility. This will enable further detailed assessment to confirm and assign pathway decisions for the patients within an acute bed prior to transfer. The post holder will be responsible for the effective coordination of patient transfers across bed based services and from within Adult community integrated teams ( therapy services) and for effective complex discharge management throughout the service. They will facilitate admissions into community bed based services from acute care / community care providers (across the south of the county) and out of area as required. To optimise patient flow and bed capacity. They will work closely with clinicians, social workers, carers, third sector - Age UK and all multidisciplinary team members to deliver a high quality patient focussed service. They will meet the required standard to proactively overcome identified concerns /issues that may delay effective discharge from services. They will coordinate assessments of patients’ needs and home circumstances and support the formation of the discharge plan. They will deputise for the Transfer of Care Facilitator as required.
