Are you a registered professional inspired to help people who need intensive short - term help at home, either as an alternative to hospitalisation, or in the transition phase when returning home from hospital? If so, we would be delighted to hear from you as these are some of the qualities we want you to bring to the CRHTT in Mid and North Hampshire. The Team maintains our focus on providing quality Home Treatment both during supported discharge and as an alternative to Hospital admission.
You would be joining a Trust that has a Philosophy of Recovery based practise, and you will have the opportunity to contribute to the working within an established CRHT Team.
We are currently in the process of expanding our services and are developing the services in both the Basingstoke and Winchester localities with separate team bases for each service
Our aim is to provide the kind of service that we would expect for ourselves, or for someone that we love and care for. A service that promotes self-direction, independence and choice, that respects the individual, and provides the treatment and help that is needed as a priority
The post-holder will develop and maintain professional supportive relationships with all members of staff with other professionals and agencies to enhance recovery and co-ordinate and monitor the delivery of person centred healthcare.
