Care Coordinator

**Description**
Gian Healthcare is looking for a Care Coordinator who will ensure the delivery of a high standard of Care across their Supported Living and Domiciliary care services in the Stockport Area.
**The Role**
Accountable to: Deputy Manager, Registered Manager and Director
Responsible for: Support Staff
**Key responsibilities**:
- Answering incoming telephone enquiries regarding client care.
- Processing new client referrals.
- Being a Liaison between service users ( and relatives), professional partners, and other stakeholders
- Carrying out Risk assessments and assisting with care plans
- Ensuring relevant matrixes are up to date at all times
- Arranging new Service User assessments and care plan reviews when required
- Scheduling Weekly care visits for our community-based staff
- Identifying capacity, bidding on and scheduling new care packages
- Contributing to our On-call / out of hours service
- Assisting senior management in the effective and efficient operation of the service.
- Assisting senior management in creating strategies that reflect the departmental staffing needs
- Providing strong leadership and support to staff
- Monitoring health and safety in the workplace and in the field
- Actively participating in the implementation, quality monitoring and improvement of systems.
- Assisting with the investigation of complaints and workplace incidents, and writing of reports.
- Providing direct support to identified higher risk clients who require support in specialist services
- An excellent understanding of the social care sector especially homecare services.
- Knowledge of the principles of a high-quality person-centered care, and anti-discriminatory care practice
- An excellent understanding of the regulatory requirements and the law relating to domiciliary care services
- Knowledge of the health and safety and Risk management practices relating to homecare services
- Knowledge of safeguarding procedures
- Successful experience of leading and managing teams delivering health and social care services
- Knowledge of all the CQC domains
**Essential**:
- Must have right to live and work in the UK
- Providing care: 2 years (required)
- Driving Licence (required)
- Ms Office Suite (required)
- Care coordinating: 1 year (Preferred but not essential)
