**Job description**

Diversity Housing is a small but growing specialist landlord.

We offer accommodation with intensive housing management to help tenants with Mental Health illness to live independently.

We currently have 21 sites across the Bradford District.

We have 1 bedroom, 3/4-bedroom shared accommodation and one 10-bedroom shared accommodation. We work alongside specialist support agencies.

**Principle Responsibilities**:
To provide direct high-quality housing and support management and practical help to high level complex needs service users to enable them to achieve wellbeing and independent living.

To engage in a supportive, meaningful and empathic manner with all residents, building trust, developing relationships and providing effective support, empowering service users to gain/rebuild confidence, motivation and resilience to maintain independent living.

To fulfil housing management and health and safety functions in line with practices and regulations.

Liaise and work closely with specialist agencies (statutory, public, private and voluntary), including health, social care, local authorities and housing in supporting residents to achieve results and their outcomes.

**Key tasks and responsibilities**

**Working with service users**:

- To help develop support plans/risk assessments to achieve outcomes through a range of interventions, including key working, 1-1 support, informal support, crisis work, accompanied appointments, signposting and a programme of informal and formal activities.
- Deliver high quality support, working with service users to define and achieve their own goals, achieving results and outcomes as identified within their support plan, enabling service users to improve their health and wellbeing and achieve independent living.
- To work creatively and flexibly with service users to remove barriers to improving their health and wellbeing and achieving independent living, including accessing services.
- Engage in a supportive, meaningful and empathic manner with all service users, building trust, developing relationships and to provide effective support.
- Empower service users to gain/rebuild confidence, motivation and resilience to maintain independent living.
- Liaise with the community events co-ordinator and to promote service user involvement and engagement through regular group activities.
- Identify any additional support needs required by residents around their complex needs and instigate support through specialist agencies.
- Liaise and work closely with specialist agencies (statutory, public, private and voluntary), including health, social care, local authorities and housing in supporting service users to achieve results and their outcomes.
- To advocate for service users on their behalf and facilitate interaction with other agencies.
- Assist service users with their finances and with debt management, including access to all welfare benefits and grants available, and to help plan how to manage their income.
- Assist the service user with start-up tenancy support and advice; assistance to set up bill payments and support housing benefits claims.
- Keep clear, factual and appropriately detailed electronic and manual records, including client records, diary records, financial systems and other records as required, to ensure all support work is properly recorded, enabling other members of the Diversity team to pick up those records and continue with appropriate support as needed.
- To identify and monitor safeguarding concerns, ensuring all relevant policies and legislation are adhered to.

**General**
- Carry out any other tasks as may be required by the Housing Manager
- Attend all mandatory training along with internal/external training opportunities relevant to the role.
- Maintain confidentiality in accordance with our data protection policy and procedure.

**Decision Making Authority**
- To be responsible for any day-to-day decisions within the confines of the duties allocated and Job Description

**Job Type**: Full time
