An excellent opportunity has arisen for an experienced Social Worker practitioner who has experience of working with Adults with a wide range of presenting needs. You will be based in a well
- supported/managed team.
You will be involved in comprehensive assessments and safeguarding work that will identify real outcomes for our customers that will enable them to remain as independent as they are able within their community.
Responsibilities comprise the following:
- Plan, organise and be responsible for case co-ordination and implementation of care & protection plans.
- Working with service users and their families/carers in one or more social work specialities
- Case co-ordination and implementation of care & protection plans.
- Working with service users and their families/carers to adopt an integrated care programme approach and risk assessment and engage with people refusing a service who are at risk to self or others
- Support people presenting in crisis needing a quick resolution, using relevant legislation and resources
- Prepare and present reports as necessary to facilitate proper decisions being made in respect of service users
- Establish appropriate professional relationships with service users, their families and other professionals to enable effective partnerships in the provision of services.
You will be Social Work England registered, ideally a driver with access to own car and able to reside and work in the UK
