Patient Co-ordinator

We currently have a vacancy for an enthusiastic and motivated Patient Co-Ordinator to join the multi-disciplinary team in Kent to support our service users. We are looking for applicants that are understanding, enthusiastic and passionate about patient care.
**Main duties and responsibilities**
A Patient Co-Ordinator serves as the link between the patients’ and the TBC Healthcare service. If a patient has any questions, concerns or comments regarding their care, the patient Co-Ordinator is the first point of contact. Their primary responsibility is the patients’ satisfaction with the services received.
The Patient Co-Ordinator will be a good communicator and able to demonstrate an ability to listen with empathy. They should be a problem solver with a “can-do” attitude, a team player and should be able to communicate with professionals on all levels.
**General Duties**
- Entering patient data on to the in-house system (full training will be provided)
- Contacting patients to introduce them to the service
- Booking appointments
- Liaising with health professionals such as GPs
- Overseeing patients’ pathways from referral to discharge
**General Responsibilities**
- To be responsible for the patient’s experience for the length of time they are in the service.
- To build relationships with and be the first point of contact to patients and to maintain excellent communication with GPs, Practice Managers, and Tier 2 services.
- To work with the Service Manager and rest of the patient care team to resolve any problems that may arise.
**Employment Type: PAYE**
**Hours: 37.5 HRS**

**Salary**: £22,000.00 per year

- 8 hour shift
