Health Care Support Worker

An exciting opportunity has arisen for a Band 3 Health care support worker to join the busy Cardiff community learning disability team. The post will work under the delegation of a Registered practitioner in the delivery of providing service users, families and organisations with the practical support and input required to ensure their health needs are met. The post holder will support service users to access primary and secondary healthcare alongside the registered practitioner.
We are looking for a person who is able to organise their allocated case load whilst ensuring they are working within their competencies. They will need to have excellent communication skills to develop links and liaise with community services and specialist organisations.
The will need to demonstrate the values of the health board and be an active part of the community learning disability team.
The main aspects of the post will to be to work directly under qualified staff to support the physical health care elements of service users are being met and to liaise.
