Governance Service
- Responsibility for the provision of a holistic clinical governance service to the Urgent Care First Response service which includes but is not limited to supporting incident reviews and investigations, governance meetings and associated administration, incident management, data analysis and reporting, learning and assurance, risk review and any other associated duties relating to Clinical Governance for the First Response service.
Supporting Investigation Managers
- To support the Investigation Managers within First Response in relation to the governance service, learning and assurance, including supporting with 72 hour reviews (Immediate Safety Assurance Reviews) and completion of patient timelines, along with supporting systems based investigations where appropriate and may include composition of certain aspects of the report, arranging meetings and ad-hoc requests for information from services.
Incident Review & Management
- Responsibility for the drafting, monitoring, processing and progressing of Immediate Safety Assurance Reviews and other incidents within First Response. To follow up with the drafting or completion of 72 hour reviews and their submission to the appropriate forums including Patient Safety Oversight Group and Immediate Safety Assurance Forum, in a timely manner and in liaison with the Investigation Managers and clinical teams.
- Responsibility for initially receiving and reviewing all incidents for First Response, and for those graded harm A and B, to draft an ISAR ready for review by both the Investigation Managers and Clinical Teams and liaise with the appropriate persons for clinical review for learning.
- To support and engage with the clinical teams in relation to incident review and management. This may include data analysis from Datix, education and training to clinical staff for grading of incidents and incident management, and use of Datix in within CWP policies and procedures in relation to best practice.
Assurance & Learning
- To liaise with the appropriate governance teams, including Business & Governance, East and West Operations and the Incidents, Inquests, Legal and Complaints Team within CWP to ensure that First Response provide a high achieving governance service in terms of procedures, compliance and assurance.
- To monitor the progress of Action Plans from investigations and liaise with the appropriate clinical teams to ensure that the Action Plans are progressed appropriately and in a timely manner and to report at various meetings on their progress.
- The role will incorporate the promotion of a positive learning environment where changes are established and embedded to improve patient safety, experience and outcomes
