Responsible for Management and Leadership within the team to support the delivery of new and existing services across Living well and Lymphoedema services. We recognise the skills required to undertake this role may require a period of mentorship and training. Purpose of Job To provide management and leadership support of the team and facilitate the integration of palliative care services by providing leadership through the delivery of the Living well Ethos across all Trinity Hospice services. Provide knowledge and expertise whilst being a clinical resource for professionals, patients and carers supported by the service.
Job purpose Leadership and Management To hold clinical responsibility for the clinics caseload of patients, and to organise this efficiently and effectively regarding clinical priorities and use of time. To provide direct line management of team members. Effectively manage attendance at work. Organise and undertake one to one meetings and PDR reviews with team members.
To represent the service, giving specialised advice to MDT, Trinity management members when required. Ensure care needs of the caseload are met by efficient team management and delegation of work to other members of the team. Support the development of new ways of working within the Living well service. Take part in audit of service provision and patient care and action plan improvements.
Compile living well reports as required. To provide oversight of the running of the clinics, management of referrals, KPIs and sickness absence and annual leave management. Play a pivotal part in the redesign or development of future service development alongside Trinity and external parties. Ability to meet deadlines.
Clinical To support the implementation of holistic management plans, using safe and effective evidence based care. Based upon clinical assessment, make autonomous decisions. To develop appropriate assessment skills in evaluating the patients condition. Attend the Living well MDT to discuss patients, carers and support staff to engage in supervision.
To formulate an appropriate, individual management and/or treatment programme, using clinical reasoning skills, setting objectives, timed goals and ensuring treatment progression with a variety of treatment techniques. Maintain accurate clinical records and manage clinic appointments. To provide spontaneous and planned advice, teaching and instruction to patients, relatives, carers, and other health care professionals, to promote understanding of the aims of the service and to ensure a consistent approach to patient care. To ensure a high standard of clinical care for the patients.
Education To provide education and support to community and hospital based teams and colleagues, including communicating changes within the service. Be responsible for seeking out learning opportunities and projects to improve service delivery. Support pre and post registration students, staff from other hospices, medical students and other healthcare professionals undertaking clinical placements. Give talks alongside the fundraising department acknowledging the role the fundraising team can support the service development.
Organisation Ability to organise time effectively, creating work schedules, prioritising workloads, and meeting deadlines. Using Initiative Able to take responsibility for own actions and make autonomous decisions. Resilience Ability to remain calm and self controlled under pressure Communication Experience of communicating effectively, both verbally and in writing, adapting style to suit the audience To communicate effectively with other members of the multi disciplinary team, about patient issues, in order to facilitate a team approach to patient care, writing to General Practitioners etc. To communicate effectively with patients and carers to maximise therapeutic outcomes, and to ensure understanding of the condition.
Communication skills in persuasion, motivation and explanation will be used. Barriers to effective communication will occasionally be encountered e.g. hearing loss, Special Needs. To assess capacity, gain consent and have ability to work within a legal framework and to support other staff to do likewise Team Working Ability to develop effective and supportive relationships with colleagues Influencing and Negotiating Experience of influencing, convincing or negotiating with others in a way that results in acceptance and behaviour change Building Relationships Ability to quickly build a rapport and establish professional relationships with others, based on trust and understanding Client Focus Ability to ensure the patient is central to all decisions made regarding management of care IT skills Ability to use Microsoft Office including Word, Excel and Outlook.
