Our friendly and supportive team, are based in Bury St Edmunds and Newmarket.

We are looking to welcome a Senior Mental Health Practitioner, to join us in providing an excellent service to Young adults and their families, in West Suffolk.

You’ll be working alongside a close knit multi-disciplinary team of, Nurses, Social Worker and Psychiatrists as well as working closely with Secondary Mental Health colleagues, GP surgeries, PCNs, Social Workers within ACS and Care Agencies.

We are an established team with a high retention rate, and we are looking to welcome a Senior Mental Health Practitioner. You will be given full support and a comprehensive induction.

We are looking for someone who is interested in working in Young People’s services, has an interest in physical as well as mental health and who understands assessment, the referral process, case handling and working collaboratively with the wider MDT.

As a Community Mental Health Practitioner, you will be responsible for:

- Holding a caseload of service users
- Assessing and managing risk.
- Using Dialog+ where possible and Recovery Care Planning with service users and carers.
- Managing your own diary and planning your day.
- Attending MDT meetings and wards.
- Participating in the Duty Rota.
- Communicate effectively through notes on Lorenzo and letters to Service Users, GPs and other involved parties.
- Driving is essential for this role.
