If you answered yes to any these questions, then Meadowfield are looking for a Carer Support Lead to join us on the journey to providing outstanding care and support to service users, carers and their families in West Sussex.
Meadowfield is an acute mental health hospital in Worthing. We are an open unit, and our open doors are something that we are very proud of and this brings a very different feel to the unit than those that are not able to use this approach.
The carer support lead for Meadowfield will complete and monitor 72 hour carer phone calls, ensure the distribution of carer packs, book in carer meetings, lead on and promote the monthly carer group at the hospital. They will lead on the carer strategy for the ward and will be supervised monthly.
You will update and collate information to keep our patient information current and compliant.
You will be a dedicated resilient team player with the ability to work well under pressure whilst displaying excellent organisational and interpersonal skills
We welcome applicants who have lived experience of caring for a loved one with a mental illness to support the delivery of compassionate interventions. You will provide peer support and practical assistance to carers, families and loved ones by using your own lived experience, empowering them to fully engage with our service Triangle of Care, to regain control over their lives and meet their own needs.

- Provide carer support to carers, friends and families of those admitted to Meadowfield.
- Ensure all 72 hour carer phone calls are completed and followed up.
- Ensure all carers receive a copy of the wards carers pack, including our carer letter.
- Invite carers to ward reviews.
- Lead on the monthly carers group, ensuring carers are invited and followed up.
- Document carer input on the Carenotes system and ensure information is circulated to team.
- Refer carers to Carer services and support.
- Build links with local carer charities, such as Carers Support West Sussex.
- Create a relapse prevention and support plan with the carer.
- Initiate and follow up referrals to carer support.
- Monitor the progress of the carer strategy and link into the triangle of care lead.
