Greenwich Primary Secondary Care Interface Clinical

Clinical and Care Professional Leads support south east London-wide and borough programmes of work, therefore, there is an expectation that you currently work within south east London, and for borough based roles, within that borough.
The main purpose of this role is to provide clinical and care professional leadership within the Healthier Greenwich Partnership and support the ICB at Place to address the 4 specific interface domains detailed within the NHS England » Delivery plan for recovering access to primary care
- Onward referrals, Complete care, Call and recall, Clear points of contact
The role will be the go-to point for all thematic queries and concerns relating to the primary and secondary interface locally. The focus of this work will be to build relationships across the interface and to identify and prioritise key issues. Specific actions will be delegated to existing teams/groups who are best placed to deliver the solutions
The post-holder will work alongside colleagues from primary, community and secondary care, and commissioning to develop the necessary relationships to support the improvement in the interfaces.

- Develop and expand ‘Place’ or ‘Trust-based’ interface groups to define and address more local issues with a focus on building relationships (to include Lewisham and Greenwich Trust, and Oxleas NHS Foundation Trust)
- Represent the Healthier Greenwich Partnership as part of an interface group at ICS level with the remit of tackling common and recurrent issues across the system which will include sharing of learning and best practice from Place.
- Develop and prioritise a workplan to address the issues and challenges raised through these forums and lead local system Task and Finish work to agree and embed effective and sustainable solutions.

- Provide an effective conduit between Primary Care and Secondary Care within Greenwich on systemic and ad hoc issues and concerns and liaise with the Primary Care Directorate in relation to solutions.
- To act as a key point of contact for planned care-related issues and opportunities originating within the borough’s constituent GPs, community services and third sector partners.
