**Job Description**.**:
My client, a Northwest ICB requires an interim CYP CHC assessor to start immediately to cover a period of integration and change.

Day to day duties:

- Provide support for Children and Young Peoples continuing care assessments
- Responsible for new referrals, transition cases and reviews
- Managing complex caseloads, ensuring thorough assessments
- Responsible for brokerage and care package initiation and dealing with complaints and appeals
- Present cases for monthly panels and agree bespoke packages
- Support children with various care needs to include care at home, in school, hospice, voluntary care organisations and school holiday clubs
- Contributed to and influence the management of specific care groups of children and young people
- Undertake co-location working with colleagues in SEND and Social Care Management
- Support family friendly services and other community services
- Provide clinical expertise and knowledge to support the decision-making process
