Would you like to join us in the Specialist Palliative Care team? We are a multidisciplinary team of expert clinicians and support staff who support people at the end of life. Our focus is assessing and supporting patients who are at the end of life. The aim of the service is to ensure patients have the best possible care at the most difficult time. We are looking for an inspirational and dynamic Clinical Lead, who would like to further develop their clinical and leadership skills.

If yes, we have a fantastic opportunity to join the Integrated Community Service Division as Clinical Lead. We are seeking an individual of high calibre, with specialist clinical knowledge and skills in palliative care and clinical practice experience, that is both inspirational and highly motivated to support the delivery of high quality care through clinical leadership. We have some exciting developments over the next 3 years, in relation to developing our teams structure, the introduction of ACP's and improved 7 day cover. The Clinical lead role will be key to developing these work streams.

In this role you will be required to work closely with our Medical Lead and Neighbourhood and specialist service lead to provide both clinical leadership and strategic direction. A proven ability to engage clinically is essential.

Provide overall clinical and professional leadership to the team in collaboration with the senior management team.

Work with the SPC workforce in further developing services for the local population, preventing inappropriate admission to hospital and delivering care closer to home.

Be a catalyst for strengthening and developing the wider palliative care agenda throughout Bolton FT and the wider Bolton stakeholders and clinical excellence to all those involved in delivering end of life care to the citizens of Bolton.

Drive the agenda to improve outcomes for patients by benchmarking and improving quality and services. A key part of the role will be to continually strive to develop quality patient focused palliative care services in line with the National Standards and NICE Quality indicators across the Bolton system

Provide a strong service development, quality improvement approach in order to drive quality improvements and outstanding, responsive end of life care for the people of Bolton.

Be directly involved with developing and embedding the Bolton strategic direction contributing to policy and clinical service development for specialist and palliative care.

If successful, you will join the Integrated Community Services Division and become a crucial contributor to the development of our Integrated Care Partnership (ICP). Working across acute and community sites. The ICP is a partnership made up of the different providers of health and care in Bolton. Together they will provide the most appropriate services for local residents in order to help them stay well for longer and in their own homes.

The aims of this ICP are to improve the health of our people and provide the best possible care to the population in which we serve and ensure that our staff have joy and pride in their work.
