We are looking for likeminded individuals who share our desire to continuously improve patient-centred outcomes and drive high quality, personalised care to join our ward teams at the Queen Elizabeth Hospital, Gateshead as part of an improvement initiative.

The Trust is currently looking to invest in highly-proactive and engaging individuals to undertake the role of Discharge Coordinator on several of our medical ward areas across the hospital.

As part of the Trust’s improvement initiative, successful applicants will be regularly consulted with to offer feedback on how patient discharge plans are progressing and how operational processes can be changed to drive better outcomes for involved.

These posts are advertised on a fixed-term basis to March-23.
- To coordinate the day-to-day process of patient discharge at ward level.
- To advise and assist ward teams with simple and complex discharges.
- Attend daily board rounds and MDT meetings to ensure the medically fit status of all patients are recorded and accurate discharge plans are in place.
- Proactively encourage and/or plan all discharge activities.
- To proactively chase and liaise with the relevant teams when the discharge is dependent in a specific action, i.e. reviewing blood result.
- Raise and process relevant and timely referrals as and when required to assist discharge planning.
- As the central point of contact for discharge planning provide regular discharge updates to patients, family members, ward teams, internal and external support services/agencies.
- Arrange the details of any follow-up care.
- Provide patients, their families and carers with details of who to contact about medicine and equipment problems that occur in the 24 hours after discharge.
- Complete any admin and data collection relating to the ward incl. f&f tests.
- Promote shared understanding of discharge processes and requirements to wider team(s)
- Supporting Trust-wide improvement initiatives such as implementation of weekly long length of reviews and ward ways of working.

Based in the North East of England we provide a range of hospital and community health services from our leading facilities including the Queen Elizabeth Hospital, Blaydon urgent care centre and Bensham Hospital, all within Gateshead.

More than £250 million has been invested over recent years in major new cultural and leisure facilities along the river bank. Gateshead is a pleasant combination of urban development in the east and rural land to the west. Some of the best coast and countryside in the country are all easily accessible from Gateshead. The area is well served with public transport, and air and ferry links to mainland Europe are both about seven miles away.

And of course, Gateshead is home to Anthony Gormley’s world famous multi-award winning sculpture, the Angel of the North. This major landmark at the entrance to Gateshead is Britain’s largest sculpture and is seen by more than 33 million people every year.
