**Care Assistant - Crisis Response & Hospital Discharge Service - Guaranteed and Contract Weekly Hours**
**Routes Healthcare have a fantastic, exciting new opportunity for you to work alongside the NHS in our new Routes Criss Response & Hospital Discharge Service to support patients waiting to leave hospital and get back into their own homes or helpingto keep them out of hospital.**
**Being an experienced care assistant or worker you will be responsible for enabling patients to regain their independence and encouraging each individual to reach their full potential. This is about doing care with people not for people.**
**As an experienced Healthcare Professional you will be supporting the patient with all home/personal care needs. You will be working alongside other Routes experienced staff and Healthcare professionals to identify the person’s goals and buildinga robust programme plan to meet their outcomes.**
**Full Time/Fixed Term 6 month Contract(upto)**
**37.5 hours per week, mileage and travel time paid.**
**Locations:Chorley, Greater Preston, South Ribble, _Fylde and Wyre, Skelmersdale and Ormskirk._**
**Shift Patterns: **Monday to Sunday, various shift patterns available, weekdays, weekends and evenings,**
**Salary: £22,723 pro rata, per annum. (you will be paid on a weekly or monthly basis )**
**Guaranteed and Contract Weekly Hours**
**Due to the nature of the role, as you will be supporting patients in their own homes, all care assistants must have a valid full UK driving licence and access to your own car.**
