Royal Berkshire Hospital Psychiatric Liaison Team is an established CORE 24 service as outlined by NHS England. The team is also accredited with the Royal College of Psychiatrists. The role of the Advanced Mental Health Practitioner within this service is to provide an assessment service to people who are referred to the psychiatric liaison service from the Emergency Department and from the acute admission wards at Royal Berkshire Hospital.
The post holder will assess new service users to the service, who present to the Royal Berkshire Hospital Emergency Department or patients admitted on the General Acute wards within Royal Berkshire Hospital. The inclusive age range is 16 years and above.
The post holder will be responsible for the formulation, execution and evaluation of a care plan, including signposting to other services. The post holder will ensure that each individual plan is developed in consultation/agreement with the service user and informed by the wider multidisciplinary team.
The post holder will operate within a multidisciplinary team and will be responsible for the delivery of care, risk management and evaluation of care, emphasising individual service user's recovery.
The post holder will be confident in taking positive risk in accordance with the Trust Policy around Risk Assessment and management.
