More detail about the role An exciting opportunity has arisen for an experienced Patient Flow Co-ordinator to join our team. The post holder is expected to provide efficient and effective flow coordination, working alongside our Medical, Nursing and Site Management teams to ensure that effective discharge planning and communication are maintained in order to provide a smooth, coordinated journey for the patient from admission to discharge or transfer to a base-ward. The post holders will be proficient in dealing with confidentiality issues and able to provide excellent customer care service, ensuring patients details are accurately recorded and the relevant documentation is created to support safe patient care.
