You will have a good theoretical, practical and procedural knowledge of social work, and a good understanding of the necessity of working withpartnership agencies including health, housing, education and statutory, voluntary and independent agencies.Adult Community Teams - Locations: Crawley, Horsham, Worthing, Bognor RegisThe Adult Community Teams work with people who have experienced cognitiveand/or physical disabilities over the age of 25 and primarily with older people. The strengths-based approach is embedded within the service to work with individuals to explore their networks of support in the local community and maximise independence.Theteams have excellent working relationships with health colleagues through the proactive care initiative, where social work representatives regularly factor into local GP surgery multi-disciplinary meetings. Social Workers have a varied workload which includesassessments, reviews, safeguarding enquiries and mental capacity act assessments.Hospital Teams-WorthingOur busy Hospital Teams support adults across a range of ages with physical disabilities, acquired brain injuries, chronic health conditions, older peopleand people with dementia. The Social Workers have fostered excellent working relationships with the multi-disciplinary teams in the hospitals to ensure that people have the most appropriate support when discharged.Our progression pathways allow individualSocial Workers to choose their own career path, so they can make the most of their experience while working in West Sussex.All of our Social Workers are provided with the opportunity to progress from qualified Social Worker to Senior Social Worker througha portfolio route, and are provided with a framework of support, workshops and assessment in order to achieve this.We value our Social Work staff and have a track record of supporting further progression to senior practitioner and management roles, providinga management development programme to support this.The learning and development offer for Social Workers includes a comprehensive programme of internal and external opportunities.
