Doncaster, United Kingdom Rotherham Doncaster and South Humber NHS Foundation Trust Full time

FOLS aims to reduce or safely manage behaviour amongst people with a learning disability (LD) and/or autism which may put members of the public at risk and would otherwise lead to contact with the criminal justice system or admission to a secure hospital. We have a strong focus on prevention and early intervention, systemic working, and enhancing forensic expertise in community services so that safe community living is achieved, and secure learning disability beds reduced. Practitioners work to support the development of new pathways of care that lead to safer outcomes for patients, and to mitigate harm to potential future victims. We achieve this through six core functions:
Specialist assessment of risk and protective factors, and guidance regarding management of risk in the community,

Delivery of offence specific interventions,

Case management support to Local Care Coordinators,

Support and training to other agencies providing day to day support to this group,

Consultancy and advice to system partners,

Forensic Practitioners have significant autonomy in managing their varied caseload and developing the interventions that are relevant for each person they work with. All people on the caseload have a Care Coordinator from a learning disability team. People may be in hospital and transitioning to the community as part of Transforming Care; pending release from prison; in supported living accommodation; living independently or with families.
