**Job Title**: Wellbeing Support Practitioner
**Salary**: £12.50 p/h
**Location**: South Tyneside
**Position Type**: Full-time (38 hours per week)
**Shift Pattern**: 4 days one week, 3 days the next, working 12-hour shifts from 08:00 to 20:00

**What We’re Looking For**:
We’re seeking compassionate, dedicated Wellbeing Support Practitioners to join our team. If you have experience supporting adults with complex needs, understand the impact of trauma on daily functioning, and want to make a real difference, we’d love to hear from you.
**Your Rota**:
- Week 1: Monday, Tuesday, Saturday, Sunday
- Week 2: Wednesday, Thursday, Friday
**Your Hours**:
- Monday - Friday 08:00 - 20:00,
- Saturday & Sunday 08:00 - 12:00, 16:00 - 20:00
**Key Responsibilities**:
- **Person-Centred Care**: Develop personalised care plans, ensuring service users' voices are central to decision-making.
- **Promote Independence**: Support individuals in building daily living skills, helping them live as independently as possible.
- **Community Inclusion**: Facilitate access to social and recreational activities, encouraging participation in community life.
- **Emotional Support**: Provide emotional and social support, promoting positive mental health and well-being.
- **Advocacy**: Ensure service users have a say in decisions affecting their lives, supporting informed choices.
- **Health and Wellbeing**: Administer medication, assist with healthcare appointments, and promote healthy lifestyles.
- **Record Keeping**: Maintain accurate documentation, ensuring compliance with care standards.
**Essential criteria**:
- Experience in a care setting, ideally with adults with learning disabilities, autism, or mental health conditions
- Strong communication and interpersonal skills
- Compassionate, patient, and understanding of complex needs
- Ability to work independently and as part of a team
- Knowledge of safeguarding principles
- Basic understanding of personal care routines and medication administration
- A valid driver's license and access to a vehicle
**Desirable criteria**:
- NVQ Level 2 or 3 in Health and Social Care
- First Aid certification
- Experience planning recreational activities
