We have a comprehensive understanding of this challenging but highly rewarding sector, this enable us to place the most suitable candidates with clients and find the best opportunities for candidates.We are advertising this fantastic opportunity onbehalf of a local authority in Cheshire East who are looking to recruit a Qualified Social Worker to be part of the Community team workingJob Requirements Carrying out and review Needs Assessments & Care and Support Plans for individuals and their carer'sin a timely and effective way ensuring that they are involved fully in the process. Determine eligibility under the Care Act 2014 Working closely with primary care/community services/hospital teams/voluntary community services. You will liaise with other professionalsand carry out joint needs assessments as appropriate. Ensuring assessments and care and support plans are person centred, asset based, and outcome focussed, concentrating on what the person with care and support needs, or carer can do. Maintaining accurate,timely and person-centred records of actions and decisions and to ensure that the evidence base for decisions making is clearly justifiable. Ensuring that the outcome of all work with adults in need of care and support, and with their Carers, is to promotetheir wellbeing.Candidate Requirements: Degree in relevant professional or equivalent qualification e.g. Certificate of Qualification in Social Work or Diploma in Social Work.
