The Primary Care Network Mental Health Team (PCNMHT) is a newly developing service which comprises of three pathways. The aim of the team is to bridge the gap between primary and secondary mental health care: providing accessible primary care level mental health interventions to the population of North Lincolnshire.

**The pathways are**: Routine Pathway, Dementia Pathway and Severe Mental Illness Physical Health Check pathway. This vacancy is within the Dementia Pathway, but we are an integrated team, so awareness of all pathways is required.

The Dementia Pathway offers a step-down service from Secondary Care for patients who are diagnosed with dementia and require ongoing reviews. Part of this role will also be to triage and offer initial assessments for older adults who require a routine mental health assessment within Primary Care.

The North Lincolnshire PCNMHT are committed to establishing our service within the wider Primary Care Networks and have colleagues embedded in GP surgeries. Expanding this is the focus of our service. We welcome applicants who feel they have the drive, skills and expertise to continue this further development.

This is an exciting opportunity for an experienced Registered Mental Health Nurses (RMN) to join our team.

This role will involve offering reviews for patients within the Dementia Pathway, in accessible locations across North Lincolnshire. Networking between Primary Care and Secondary Care Services is a central part of the role to ensure the patients journey between services is seamless and care and treatment is provided in a timely manner. The role will also involve working with the wider team to triage and offer routine mental health assessments to older adults experiencing mental health problems within Primary Care.

It is essential that the applicant can demonstrate post registration experience as a Registered Mental Health Nurse and have professional experience and knowledge of working with older people affected by dementia.
