West Berkshire Council's Adult Social Care Service is seeking a Social Care Practitioner to join our Review Team, reporting to the Review Team Leader.

**The Role**

As a Social Care Practitioner, you will be responsible for:

- Conducting reviews of existing care and support arrangements to ensure they remain appropriate and effective.
- Assessing the needs of individuals and identifying opportunities for improvement in their care.
- Working collaboratively with service users, carers, and other professionals to achieve positive outcomes.
- Providing advice, guidance, and support to individuals in accessing appropriate services.
- Maintaining accurate and up-to-date records of assessments and reviews.
- A background in social care, with experience in conducting assessments and reviews.
- Knowledge of relevant legislation, regulations, and best practices within adult social care.
- Excellent communication and interpersonal skills, with the ability to build positive relationships with service users and colleagues.
- Strong organisational and time management abilities, with the capacity to manage a varied caseload.
- Team player who is also able to work independently, demonstrating initiative and problem-solving skills.
