**Job Overview**

**Duties**
- Conduct comprehensive assessments of clients to identify their needs and challenges.
- Develop and implement individualised support plans in collaboration with clients and relevant stakeholders.
- Provide ongoing support and guidance to clients, ensuring they have access to necessary resources and services.
- Maintain accurate and up-to-date records of client interactions, progress, and outcomes.
- Facilitate communication between clients and service providers, advocating on behalf of clients when necessary.
- Monitor client progress and adjust support plans as needed to ensure effective outcomes.
- Negotiate with service providers to secure appropriate services for clients.
- Participate in team meetings and case reviews to share insights and improve service delivery.

**Qualifications**
- A degree in Social Work, Psychology, or a related field is preferred but not essential.
- Previous experience in case management or social services is advantageous.
- Strong negotiation skills to advocate effectively for clients’ needs.
- Excellent communication skills, both verbal and written, with the ability to build rapport with diverse populations.
- Ability to work independently as well as part of a team in a fast-paced environment.
- Strong organisational skills with attention to detail in record keeping.
- Empathy and a genuine desire to help others improve their circumstances.

Pay: £25,000.00-£30,000.00 per year

Additional pay:

- Performance bonus
