MCH are developing our Inpatient services and provide inpatient rehabilitation at Amherst Court (stroke rehabilitation on Endeavour Suite and Britannia Suite Intermediate Care Rehabilitation) and at Harmony House (Intermediate Care Rehabilitation).

You will work as part of the nursing team to deliver patient-centred nursing and rehabilitation care, whilst working as part of a wider MDT.

You will be fully supported in your role and work alongside our nursing and therapy team which also includes a practice development nurse, Unit manager and clinical lead nurse/matron. We are looking to support staff development and there is the opportunity for staff to go complete their full nurse training should they wish.

To work as part of the Nursing Team to deliver patient-centred nursing and rehabilitation care within Stroke/Intermediate Care within a variety of settings.

To be a qualified supportive role, bridging the gap between nursing rehabilitation assistants and registered nurses.

To participate in the ongoing assessment, and be responsible for the monitoring and delivery of patient care.

Working with the patient group to encourage self-care and promote independence by providing education and support.

To ensure the safe and effective patient care, including the supervision of support/assistant staff.
- To provide and monitor safe, person centred care to a designated group of patients, service users and families in a range of care settings under the direction of a Registered Nurse, without direct supervision in line with an agreed plan of care.
- To support the registered nurse in the contribution to on-going assessment and care planning for patients.
- To promote health and prevent ill health in patients and service users and contribute to integrated care.
- To monitor the condition and health needs of patients on a continual basis in partnership with colleagues, families and carers, referring to others for reassessment, when required.
- The Nursing Associate will actively contribute to an effective learning environment and support others in their learning.
- Nursing Associates will adhere to the Nursing and Midwifery Code of Conduct for Nurses, and nursing associates and work within their scope of practice following the professional standards of practice and behaviours for nurses, midwives and nursing associates and the standards of proficiency.

Essential

Qualifications, training, knowledge and experience
- Registered Nursing Associate (NMC, Level 5)

Communication and relationship skills
- Provide and receive routine information requiring tact or persuasive skills
- Provide advice, instruction or training to groups, e.g patients/ colleagues / students/ families.
- Provide and receive complex, sensitive information; e.g. test results; receives sensitive patient/client-related information.
- Have the ability to communicate information regarding the patient’s medical condition and individualised treatment programmes in an easy to understand, reassuring way, enabling and motivating where necessary, this includes the patient, family and carers
- Communicate with patients/clients and carers, using empathy and reassurance and where there are barriers to understanding learning to manage these accordingly, e.g. learning disabilities or l
