**The Practices in the Partnership are**:
Lilliput Surgery, Poole Road Medical Centre, Wessex Road Surgery, Heatherview Medical Centre, Fernside Surgery and Parkstone Tower Practice

Situated in beautiful Dorset with the prestigious Blue Flag beaches of Bournemouth and Sandbanks, the Jurassic coastline and a wealth of attractions and countryside to visit we are lucky to be in such a fantastic area.

We are seeking a **Health Care Assistant** **(Leg Ulcer Clinic)** To be part of the Primary Health Care Team providing high quality and forward-thinking nursing care to meet the holistic healthcare needs of the Practice population. To work to agreed Practice standards and protocols for the delivery of patient care.

**This is not a career role. You will be based at a practice and will need to have experience working in a practice. You would also need to be a qualified phlebotomist, experienced taking BP readings, ECGs and injections **-- these are deal breakers

**Salary**:£11.70 per hour.

**EDUCATIONAL QUALIFICATIONS (Essential)**

GCSE/A Level equivalent

Degree/Diploma in relevant discipline

NVQ Level 2 in Health and Social Care or equivalent

Immunisation and vaccination accredited (Desirable)

Phlebotomy accredited (Desirable)

**EXPERIENCE (Essential)**

Experience of working directly in a clinical healthcare assistant role.

Evidence of effective team working

Ability to write and maintain accurate records

Evidence of continuous professional development

Experience working within a GP practice (Desirable)

Health and Safety Awareness (Desirable)

Basic first aid knowledge (Desirable)

**COMMUNICATION SKILLS (Essential)**

Great communication skills.

Full UK driving license and own transport (Desirable)

**Key clinical responsibilities**:

- Implement treatment programmes with individual patients and/or carers in their own homes as delegated and directed. Provide high standards of care encompassing a rehabilitative approach where necessary, and report back as agreed. Actively report any changes in a patient’s condition in a timely way to the registered nurse.
- To carry out a range of delegated routine clinical duties having completed training and achieved competence including but not limited to blood pressure, pulse and temperature recording, urinalysis, blood glucose monitoring, simple wound dressings, bandaging, venepuncture, continence care, stoma care, catheterisation and non-complex reviews.
- To communicate with patients at a time and level appropriate to their needs in a manner that promotes positive health
- Use clinical knowledge to identify changing or deteriorating health conditions and alert GP or Senior Nurse to take appropriate actions in partnership with the GP and/or specialist services to treat at home and avoid unnecessary hospital admissions.
- Use IT systems to optimise the planning and delivery of the nursing service and become a champion for any new systems introduced.

**Training and development**:
Training will be offered for all the below skills, these will be delivered by local and national providers and supported by Shore Medical.

You will be invited to bi-monthly whole team training days which focus on a different area each time.
