**BASE**: GP Practices in Faringdon and Botley, Oxfordshire and some home working possible.

We’re Mind, the mental health charity. We’re here to make sure anyone with a mental health problem has somewhere to turn for advice and support; in total over 20,000 people in Oxfordshire access our services each year. With one in four people experiencing a mental health problem at some point in their lives, people need Mind more than ever. Our work includes:
- The Transitional Supported Housing Recovery Service (part of the Oxfordshire Mental Health Partnership).
- The TalkingSpace Plus psychological therapy service in partnership with Oxford Health NHS Foundation Trust.
- The Oxfordshire Mind Wellbeing Service, which is part of both the TalkingSpace Plus and Oxfordshire Mental Health Partnerships and includes:
- Five Wellbeing Centres throughout the county
- Public Wellbeing & Information Services
- Physical Activity and Wellbeing
- Benefits for Better Mental Health, a welfare benefits advice service
- Volunteering and Peer Support Opportunities
- Raising awareness and campaigning activity.
We employ over 100 staff and operate in 28 locations around the county.
**ABOUT THE ROLE AND CANDIDATE**
The Primary Care Team work with service users registered at GP practices across the county, supporting them to achieve their goals and to find the right support for their needs and desires. We are a fun, caring and dynamic team which places a high value on mutual support, sharing our wide-ranging knowledge and expertise and maintaining close working relationships, despite working in different areas of the county. There are lots of opportunities for staff to develop areas of interest when it comes to working with people; on an individual and team basis. We are truly committed to the wellbeing of our staff and this remains a high priority alongside providing excellent support for our service users.
We are looking for someone who is motivated, pro-active and supportive, with a strong commitment to person-centred work and service user involvement and is interested in being a part of a fast growing team of Wellbeing Workers from all walks of life. We are currently working from home, in response to Covid-19 and are providing telephone support to our Service Users. We are hoping to return to face to face work when it is safe to do so and will provide a mixed model of face to face and telephone work, integrated within the GP practice team.

Purpose of the Job*
Provide an effective information and options service, based on a social prescribing model, to people referred by Primary Care staff or via self-referral.
Oxfordshire Mind Primary Care Wellbeing Workers offer up to six one-to-one sessions with clients who may be experiencing mental health issues or other social or lifestyle issues that are impacting on their wellbeing. Wellbeing workers will work in a non-clinical person-centred way to encourage and enable service users to link in with existing support services, use the support available in their local community and develop tools to increase their ability to manage their own wellbeing.

Responsibilities*
The key responsibility of the Primary Care Wellbeing Worker in this role is to provide an excellent person-centred service through wellbeing sessions.
Triage
- Work directly from a daily triage list of telephone calls or eConsults from people who have concerns about their mental health or wellbeing.
- Work together with the client to agree the most appropriate next steps, e.g. signposting to another service, booking an initial assessment session (see below), or booking an appointment with a GP.
Wellbeing Sessions
- Provide initial assessment sessions and follow-up sessions as required.
- Work with individuals to identify their own hopes and goals, and support them to develop a personalised action plan to increase resilience and wellbeing.
- Review and assess progress of individuals, supporting them to complete their personalised action plans.
- Carry out follow up contact with individuals to evaluate outcomes using designated measures such as the Short Warwickshire-Edinburgh Mental Wellbeing Scale (SWEMWBS) and patient satisfaction questions.
- Telephone appointments - currently all appointments are carried out via telephone from home. As restrictions ease, this will become a mixed model of telephone work and face to face work, carried out in GP practices.
Other Responsibilities
- Collect and accurately record service user data onto an online database.
