Have you worked as a Senior Carer in an adult setting? Do you like new challenges and learning new skills? Then working as a community based Senior Reablement Officer might be for you It’s an exciting time to join our team as our Reablement Service is growing from strength to strength. Our team is supportive, creative and hard working. We are looking for people with the same level of commitment we have, to support staff to promote our customers independence.

You must be self-motivated, have a flexible approach to working arrangements and have an understanding of working in the community in an individual’s own home. Your shifts will include Saturdays, Sundays and Bank Holidays as the rota dictates.

You must have a genuine interest in promoting independence and supporting an individual to regain their confidence and skills. This may be after a period of rehabilitation in a residential setting or on returning home from hospital. We also receive community referrals for customers who may be struggling to manage at home.

The main areas of responsibilities of the post include:

- Under the guidance of qualified therapists to provide individual activities as part of the assessment and treatment programmes for the individual._
- To take responsibility for the administration of medication, the necessary recording and audits._
- To build and maintain close working relationships with our stakeholders._
- To undertake risk assessments including environmental risk assessments and medication risk assessments in customer’s own homes._
- Working without direct supervision and responsible for individual progress and reporting back to the lead person on individual’s performance, behaviours, strengths, needs, concerns, and suggested progressions._
- To liaise with other professionals, e.g., GP’s, consultants, district nurses, social workers, care agencies and other allied health professionals when the need arises._
- To deliver formal and informal supervision to Reablement Officers and perform Direct Observations._
- It will be necessary for you to be ‘on-call’ from home to support Reablement Officers on shift on a rota basis and covering sickness/holidays if applicable._
- Visiting customers in their own homes, hospital ward or Care Home to gather and record information._
- To use technology and systems safely and effectively - training and support will be provided._

You must have a full clean driving licence as the post requires travelling within Middlesbrough. You must have access to a vehicle and be suitably insured for this purpose on commencement.
