**About the Role**
This role is responsible for the development and delivery of the Care Closer to Home pathway, integration and mental health. It works in partnership with health, voluntary and community sector and service providers to design pathway and improvement programmes that turn commissioning intentions into positive outcomes for individuals. This role is focused on improvement and prevention and early intervention strategies.
It supports and aligns with the Strategy and Transformation Team to ensure best practice commissioning frameworks are reflected in ASC commissioning intentions.
Some accountablilities of this role include:
- Manage the development and delivery of the Care Closer to Home pathway, maintaining a focus on improvement and prevention and early intervention strategies across:
○ Reablement
○ Community equipment
○ Telecare
- Utilise best practice commissioning frameworks, ensuring that there are commissioning strategies and plans in place that reflect local need, priorities and maximise resources
- Develop partnership working between health, voluntary and community sector and service providers, providing cross-service or multi-contract level of engagement
- Facilitate involvement of service users, families and carers in the planning, design and delivery of services and strategies, to improve their quality and outcomes
- Support iterative and continuous service development, responding to market developments and service needs
The role reports directly to the Section 75 Integration and Partnership Lead.
