Locality lead required to provide team lead to the Heath interface team of social workers and adult social care practitioners working in Yeovil District Hospital.

The team provide a service to the front door (A&E) working as part of the multi-disciplinary team to provide advice, support and intervention aimed at preventing unnecessary admissions into the acute.

The workers liaise with hospital and community colleagues, utilising community agent and voluntary services as appropriate. The team also work as part of the multi-disciplinary team in progressing discharges from the acute under the Hospital Discharge Service:Policy and Operating Model.

The majority of people in the hospital are discharged home with no requirements for any additional services or support.

We work closely with our voluntary organisations and community agents to fully utilise community options of support. Our default position is to get people home. We use our discharge to assess service for those needing some additional support on dischargehome and will use the hierarchy of bed options for those unable to go straight home, providing reablement prior to returning home.

The workers do not assess for care and support but they are required to assess capacity under the Mental Capacity Act and make best interest decisions as required, liaising with LPAs.

The service works 8am - 6pm over a 7 day week. The staff are rota'd on to shifts to ensure service cover over 7 days. We currently have a locality lead covering the weekends across the county but it would be helpful to have someone who could be flexibleif weekend cover is required at any time.

I am looking for an experienced social worker or OT who has experience of supervising and leading a team of social care staff, preferably in a hospital setting. The work is fast paced and I need someone knowledgeable and confident to work as part of themulti-disciplinary team, challenging health colleagues where necessary and ensuring the team work effectively and efficiently to progress discharges in a timely way; maintaining focus on the person at all times.
