Sensory Independence Advisor

We are looking for a motivated and flexible person to join the Southampton Community Independence Service in the sensory team as an Independence advisor. The CIS teams are multidisciplinary integrated health and social care teams covering Southampton, visiting service users at home to carry out holistic assessments incorporating care and rehabilitation needs in the community and promoting self-management. The sensory specific role would be assessing people who have been identified with a hearing loss, or have a significant sight loss affecting one or both eyes, and may have dual sensory impairment.

You will be working alongside physiotherapists, occupational therapists, Social workers and support workers to provide individualised holistic assessments in the home environment to adults who have a range of physical health needs including some complex and long term conditions, formulating plans to assist people to maintain their independence.

Training would be provided, along with regular supervision, in-house training, and the opportunity to attend relevant courses as highlighted in your personal development plan.

You will need excellent communication and interpersonal skills and enthusiasm for working as part of a multi-disciplinary team. This opportunity is for a full time hours at 37 per week.
