**Title**: Mental Health Navigator

**Location**: The role will be a field based position where you will work flexibly whilst covering the geographical area of Grimsby with regular travel across North Lincolnshire

**Hours**: 35hrs per week between Monday to Friday 9:00am - 5:00pm, however some flexibility is required depending on service needs.

**Salary**: £21.706 to £23,900 per annum

**Contract**: 2 year fixed term

We have an exciting opportunity for a Mental Health Navigator to join our established and friendly Navigation Team.

Our Mental Health Navigation team supports people with mental illness who present at primary care with unmet non-clinical needs. This model builds on the well-established social prescribing model, offering a dedicated capacity to assist and empower people to access the right support, to manage a range of needs and social distresses that can affect an individual’s mental health and in doing so alleviates pressure on healthcare systems.

How will you make a difference?

An exciting opportunity has arisen for the right individual to be part of an initiative to create a proven new model of care to improve the lives of people severely affected by mental illness in the UK. Based in North East Lincolnshire (NEL) the Community Mental Health Navigator will aim to complement the community mental health transformation programme and produce positive outcomes and impact for service users.

Working with NAViGO, the provider of Adult and Older Adult Mental Health services and the 4 Primary Care Networks in NEL, you will be a key member of the newly integrated community mental health service offer.

Reporting into the Primary Care Mental Health Liaison Practitioner, you will be fully integrated into a multi-disciplinary team working alongside mental health and primary care professionals to support service users with personalised support, and most importantly delivering great outcomes for people using services.

You will proactively engage with service users, identify non-clinical needs and provide the relevant support and guidance required to improve the patient’s mental wellbeing and quality of life.

**This can include**:
Working as part of an integrated mental health team, with a range of other mental health and primary care professionals to ensure the most appropriate care is provided to service users

Working with partner organisations including the voluntary sector to establish pathways in to support services to meet the holistic needs of your service users

Work with colleagues to ensure friends and families of service users are provided with all relevant information and support required to maintain support for the service user

**What we are looking for**:
**To be considered for the role, you will**:
Have QCF / NVQ level 3 qualification in Health & Social Care

hold a full UK driving license and have access to own transport with appropriate insurance to cover work related travel

possess working knowledge of debts, housing, mental health, dementia, employment, and social prescribing

have relevant experience in a similar role

ability to maintain professional boundaries, even in challenging situations

ability to make a comprehensive assessment of an individual’s situation and needs; to develop, implement and review risk management plans and devise support plans to meet these needs.

**You may also have**:
Mental Health qualifications or knowledge, including therapeutic interventions

Experience of providing interventions on both in a 1:1 and group format

Lived Experience of Mental Health and/or dementia either through own experience or supporting family / friends

Experience of Systemone
