Job Overview:
**Responsibilities**:

- Develop and implement personalized care plans for patients under the guidance of healthcare professionals
- Assist with medication administration and treatments as directed
- Monitor and record patient vital signs, promptly reporting any changes to the healthcare team
- Provide support and guidance to patients and their families regarding health conditions and treatment protocols
- Collaborate closely with nurses, physicians, and other healthcare providers to ensure coordinated patient care
- Maintain accurate and up-to-date patient records in compliance with regulatory standards

Qualifications:

- Previous experience in a healthcare setting preferred
- Excellent communication skills in English, both verbal and written
- Familiarity with care home or home care environments is advantageous

Nice-to-Have Skills:

- Experience working with care plans
- Ability to effectively communicate in multilingual settings

**Job Type**: Part-time

Pay: £11.44-£12.40 per hour

Expected hours: 30 - 45 per week

- Mandatory Trainings and Mapa training certificate (required)
