**Job Summary**
We are seeking a compassionate ,dedicated and Experienced Healthcare Assistant to join our team. In this role, you will provide essential support to patients in a care home or home care setting, ensuring their comfort and well-being. You will work closely with healthcare professionals to implement care plans and assist with daily living activities, fostering a supportive environment for our clients.

We are a CQC Outstanding rated Care Home with Gold Standard Framework accreditation in end of life care therefore your knowledge and experience of care need to be at a high level.

**Duties**
- Assist patients with daily living activities such as bathing, dressing, and meal preparation.
- Support the implementation of individual care plans tailored to each patient's needs.
- Communicate effectively with patients, their families, and healthcare professionals to ensure continuity of care.
- Maintain accurate records of patient progress and any changes in condition using IT systems.
- Help with mobility assistance and transport patients as required, including driving if necessary.
- Ensure the cleanliness and safety of the care environment, adhering to health and safety regulations.
- Participate in training sessions and professional development opportunities to enhance skills and knowledge.

**Experience**
- Previous experience in a nursing home healthcare setting is essential
- Strong communication skills in English are required for effective interaction with patients and team members.
- Familiarity and experience with digital care plans is beneficial.

Pay: £12.85-£14.00 per hour

Expected hours: 36 per week

Additional pay:

- Yearly bonus
