- Mon-Thurs 8.30 - 5.00 Friday 8.30 - 4.30Working Pattern: Flexibility around the needs of the service but will need to be in the office at least 3 days and will also have to make hospital visitsThis remit also includes service users who have a diagnosis ofmental illness and Autism/ mental illness.To complete assessments, to supervise care and support plans, advice on complex cases, risk manage.To case manage complex cases, complete care and support assessments/plans, reviews and commission safe cost effectivepackages of care and placements usually via Direct payments.To attend the multi-disciplinary team meeting to promote best practice and joint working.To fulfil the statutory requirements of RCC under the Care Act 2014.To undertake safeguarding enquiries andconvene multi agency meetings around vulnerable adult risk management meetingsTo work with our legal department on cases where ordinary residence and Sec117 aftercare entitlement are in dispute.To complete reports for Mental Health Tribunals and managers hearingsand participate in safe discharge planning. This will involve visiting inpatient sites.Support with adult duty and add to the capacity of adult social care.Qualified Social Worker Requirements
- CSW/CQSW/DipSW/BA/MA in Social Work
- Proven post qualifying experiencein working with Adult services
- Current Social Worker England registration
- Current Enhanced DBS Disclosure
- Eligibility to work in the UK without work permit or visa restrictions
