About this job:
The project is a resource and activity centre, which incorporates a drop-in facility. In addition, the Adferiad Recovery project facilitates outreach groups in the County. The Adferiad Recovery projects promote positive attitudes towards mental health in its broadest sense by providing a range of services, which will help to prevent the development of mental health problems. It also provides support to people who are recovering from a period of mental ill health, their families, friends and informal Carers.
The project, staff and volunteers aim to empower individuals to manage their mental well-being by utilising the project, groups and individual support sessions as a means of achieving the goals agreed within their individual recovery plan. Individuals receive emotional and practical support, which can include daily living skills / housing related support from the Adferiad Recover staff when attending either the TRI centre or organized groups, the overall aim is to enable each person to continue living independently in the community and access other resources.
Main Duties and Responsibilities:
Team Management
- To work closely with the Service Manager and or County Manager.
- Lead on performance of the staff and volunteers; allocate tasks, agree objectives, set out plans and work methods, monitor and evaluate performance to ensure all service contract objectives are met.
- The Project Co-ordinator is to work with the staff, volunteers and members identifying and developing community links which will benefit the individuals the project supports, within their own communities.
- Monitor and evaluate staff and volunteer’s performance during supervisions and with ongoing support.
- Identify and support the role specific training and development needs of staff.
- Develop and maintain productive working relationships with Adferiad Recovery colleagues at a local and national level.
Service Delivery
- Following procedures for processing and assessing referrals, completing new assessments, risk assessments and creating individual person-centred recovery support plans with the individual.
- Deliver a range of activities, training, support sessions and co-ordinator the project.
- Support individuals in a range of activities on an on-site and peripatetic basis.
- Providing care and support services to individuals or groups of individuals.
- Promoting, encouraging and facilitating the use of local community facilities.
- Provide advice and refer people experiencing mental distress to mental health services, liaising with mental health teams as necessary.
- Ensure clients are aware of their rights and responsibilities when engaging with Adferiad Recovery and society more generally.
- Supporting and assisting attendees of the centre / outreach groups with the following:
Feeling secure within their home
Increasing independent living skills
Personal finance, budgeting, and accessing benefits
Mental and physical healthcare
Building and maintaining relationships with neighbours, families, friends, professionals
Developing skills and interests
Pursuing training/educational/employment goals and aspirations
Pursuing leisure/recreational goals and interests
Developing links with the local community-based resources and opportunities
Signposting, joint work and referral on to other specialist agencies, intervening in crisis situation
Recognition of and coping strategies in respect of risk, triggers and avoiding relapse
Promoting recovery.
- Follow procedures for conducting 6 monthly and annual reviews - or as and when required
due to changes in individuals’ needs or circumstances, ensuring risk assessments and support delivery plans are kept up to date and are relevant.
- Completing and recording Outcomes and producing monitoring reports for internal/external purposes.
- Keeping updated with changes to Outcomes data collection.
- Ensure external and internal communication is clear, timely and of high quality.
- Data collection and appropriate recording and presentation for monitoring and Quality Assurance reports as directed by the Service Manager / County Manager.
- Follow procedures for general record keeping and information collection, saving and archiving - adhering to confidentiality and data protection policies.
- Actively seeking opportunities to promote the services offered by the organisation within the local community, and potential sources of referral. Including attending events for this purpose and the distribution of locally produced publicity material.
